Origins Counseling’s Diagnostic Assessment Program can give you answers. We are here to help you understand both what is happening and exactly what to do about it. Maybe something just feels “off” with you or a loved one. Feelings of increased or constant stress, trouble sleeping, a sense of sadness or disconnect from the world, feeling overwhelmed, lacking a sense of purpose; all of these can be symptoms of an underlying problem that can be understood and helped.

Basic Consultation

A personalized, confidential mental health consult with a masters-level therapist

For those unable to make it into an office, Origins offers a brief consultation that can be completed virtually. During this 60 minute consultation, a licensed therapist will work with you to explore the issues that are most important to you and will provide you with recommendations for your next steps. Sometimes these involve further assessment, lifestyle change, engaging professional or peer support, or a host of other possibilities.

After the review, a written summary is provided to you by secure email.

Standard Assessment

If you are looking for a quick and flexible solution to help you assess your mental health or substance use, our standard assessment can help. We complete a detailed diagnostic from psychological testing and clinical interviews to determine a path for your wellness.

Your Standard Assessment includes:

  • Completion of an initial questionnaire and screening tool that you can complete over the phone or online in the comfort of your home (10-15 minutes).
  • Face-to-face interview with a mental health professional (MHP) who will help guide you through a process of discovery to really get to the root of what is happening. The MHP may also have you complete additional assessments as part of this process based on your specific needs (1-2 hours).
  • Follow-up visit with MHP to discuss findings and recommendations which will be provided to you in a written report. Recommendations will provide you with immediately accessible options, not just ideas and ideals (1 hour).

 

Comprehensive Assessment

Sometimes, we may have a good idea of what the problems are but just have no idea what to do about them or if they are really even all that bad. You or someone you love is doing or experiencing something and it is causing problems in life. Substance use, gambling, sexual activity, gaming, relationship with food, emotions, impulses; all of these have the potential to be areas of grave concern when they are not healthy. Origins Counseling’s Discover Level 2 assessment helps you really hone in to understand how severe a problem might be and what kind of care will give you or your loved one the best chance at lasing health and wholeness.

This assessment is also appropriate for individuals who may have been asked by their employers or the legal system to complete an assessment following some incident.

Your Comprehensive Assessment includes:

  • Completion of an initial questionnaire and screening tool that you can complete over the phone or online in the comfort of your home (10-15 minutes).
  • Urine drug analysis (UDS).
  • Baseline neurocognitive and personality assessments (1-3 hours).
  • Face-to-face interview with a mental health professional (MHP) who has reviewed your screenings and may provide an additional assessment based on initial tests. From here the MHP will join with you in a guided interview that will explore the problem that brought you in but, more importantly, help to illuminate you as a whole person (2-3 hours).
  • For assessments that involve a return to work or legal purposes, you may provide references that the MHP will speak to by phone to provide additional perspective or confirmatory information.
  • A comprehensive report provided to you in the digital format of your choice that includes:
    • Testing results
    • A summary of your interview with the MHP
    • Clinical recommendations

  • A follow-up visit with MHP to discuss findings and recommendations. You may wish to include others in this session and, as this is often very helpful, you should discuss this with the MHP beforehand (1-2 hours).
